Award Orders

Using the “2025 Award Order Form” button below, you can order plaques, statuettes and extra certificates.

The order deadline is Friday, April 11.

Payment is due Friday, April 18.

Whatever your award level, you can choose how you want to commemorate it. All shows receiving an award will be recognized with a complimentary certificate automatically. You do not need to request a complimentary certificate.

How do I get my award?

We hope you will attend the Best of the Midwest Media Fest Banquet and Show on May 1 and accept your award in person. If you cannot attend, we will make arrangements to get your award to you. We will not ship plaques or statuettes free of charge. More information is on the order form. You can also select someone to pick up your award for you. You’ll put that name on your order form.

What do the awards look like?

Plaques are 7” x 9” and the clear plexiglass statuettes are 8.25” tall. The color of the metal plate on the plaques changes depending on the award. Green is used for Merit Awards, red is used for Achievement Awards and blue is used for Excellence Awards. Significant Community Programs plaques use a black metal background.
